Acute dentoalveolar abscess with blood extravasation and its association with trauma due to misuse of the Essix retainer. A case report.

Abstract

Excessive forces exerted on the teeth can give rise to endodontic conditions. A misaligned Essix-type removable retaining appliance could generate undesirable effects that could result in pulpal necrosis. However, there is not enough evidence in the literature that post-orthodontic removable retaining devices can cause irreversible pulpal lesions. The following report presents the clinical case of a 31-year-old patient who reported acute, localized, and pulsating pain near the tooth #2.3 with 48 hours of evolution after the inappropriate use of a misaligned Essix retainer. Diagnostic methods such as anamnesis, vitality tests and radiographic images were used and the existence of an acute periapical abscess with blood extravasation was determined. As a treatment plan, a root canal treatment was carried on and periodic evaluation at the first, third and sixth month were performed, which confirmed the reduction of the lesion and recovery of the surrounding bone tissue. The objective of this report is to present the clinical case of a dentoalveolar abscess caused by trauma when using a misaligned post-orthodontic retention device.
