The extraction of the first permanent molars is considered an atypical surgical procedure, due to the fact that its removal is not usually indicated when treating a skeletal Class II, except when there is a great coronary destruction caused by caries at an early age. We present the case of a mesofacial adolescent patient who was proposed for the extraction of the upper first permanent molars to resolve a skeletal Class II with an overjet of 10 mm, where the presence of the third molar was vital to replace the second permanent molar, allowing thus, the resolution of the clinical case as if it were a case without dental extractions that was carried out conservatively.
